Every time you need to go to an eye care provider, just follow these steps:
- Choose a Blue View Vision network eye care provider. With over 44,000 providers and provider locations including Lens Crafters ® , Target ® Optical, Sears ® Optical, JCPennys ® Optical and Most Pearle Vision ®. You may find additional locations at anthem.com.
- Make an appointment.
- At the time of your office visit, present your health/vision ID card, and pay your copayment and any balance for non-covered services.
Your network eye care provider will verify eligibility and handle the necessary paperwork.
