  1. Which plan is the best one?
Our high option Plan PPO provides superior dental benefits. The Plan HMO is excellent as well and represents a very good value.
  1. When will my dental plan benefits go into effect?
You will be eligible the 1st of the month after we have received three pay periods or one months premium.
  1. How long do I have to remain on the plan?
Your enrollment period is for one year periods at a time, and automatically renewed for one year periods.
  1. What is the difference between the "Preferred Provider" plans and "Dentist of Choice" plans?
The "Preferred Provider" plans provide you with access to our select panel of dentists. These dentists have agreed to charge reduced fees and account for big savings for our customers. When a member goes to the "Dentist of your choice" plan, that dentist may charge their usual and customary fee, leaving you to pay the remainder.
  1. May I change my preferred provider dentist?
Yes. Simply notify us and you may change dental offices at any time.
  1. What forms of payment can I remit my premiums with?
You may pay your dental premiums through:
  1. Bank Authorization plan. A monthly deduction out of your personal checking account.
  2. Annual payment. Premiums are paid annually at the beginning of each years enrollment.
  1. What are the costs of the plan?

For the Plan PPO:

  • Members only – $85.00 per month
  • Member & spouse – $120.00 per month
  • Member & child – N/A per month
  • Family – N/A per month

For the Plan HMO:

  • Members only – N/A per month
  • Member & spouse – N/A per month
  • Member & child – N/A per month
  • Family – N/A per month
