Disability Plan
Enrollment and Coverage Effective Date
If you are eligible and enroll in the C S A L C. Disability Plan, your coverage will become effective on the first day of the month following the date of approval, provided the first months premium has been paid and provided you meet al1 eligibility requirements.
If you are absent from work due to an injury or sickness on the date your insurance would normally become effective' your coverage will begin on the first day of the month after the date you have returned to active full-time status and have completed 30 full days of work.
Your coverage will continue for as long as 1) the group policy stays in effect, 2) your premium payments are made through allotment from your paycheck and 3) you remain an eligible employee.
NOTE: If the group policy terminates while you are receiving disability benefits, your payments will continue for as long as you are disabled and entitled to benefits under the plan.
